About the position

The Case Manager at Advanced Therapy Associates, part of the Optum family, plays a crucial role in providing resources, education, and support to clients. This position is integral to the Optum Behavioral Care Team's mission to enhance healthcare delivery for vulnerable populations by developing and implementing individualized care plans that address the physical, mental, and social needs of patients.

Responsibilities

• Provide education and support to families
• Coordinate services within and outside of the clinic
• Help clients access and utilize community resources
• Act as a liaison between the client and service providers

Requirements

• Bachelor's or Master's degree in social work, psychology or a related human services field
• Experience with case management or other mental health related experience
• Proven solid verbal and written communication skills

• Experience with couples and family counseling
• Experience with children or adolescents
• Experience with health psychology
• Competent with a wide range of clinical requisitions
