About the position

The Certified Case Manager (CCM) plays a crucial role in the interdisciplinary team, managing and directing resource utilization to ensure high-quality outcomes during a patient's rehabilitation. The CCM coordinates patient care from admission through post-discharge, advocating for the patient and facilitating communication among healthcare providers. This position involves overseeing the discharge plan, addressing psychosocial issues, and ensuring compliance with regulations regarding patient rights.

Responsibilities

  • Coordinate and advocate for patients during hospitalization and post-discharge.
  • Manage information to oversee healthcare delivery and facilitate interdisciplinary care decisions.
  • Establish and monitor the implementation of discharge plans.
  • Identify and address patients' psychosocial and support system issues.
  • Oversee effective coordination of services related to admission and discharge processes.
  • Facilitate team conferences and communication regarding the interdisciplinary plan of care.
  • Educate patients and families about care processes and resources.
  • Manage payor relations and fiscal management of patient care.
  • Perform ongoing utilization reviews and act as a liaison to payors.
  • Ensure compliance with regulations regarding patient rights.

Requirements

  • Must be qualified to independently complete assessments within the scope of practice (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
  • Active licensure required if applicable to discipline.
  • Current CCM® or ACM certification is required.
  • For Nursing, a minimum of an Associate Degree in Nursing is required; BSN preferred.
  • For other healthcare professionals, a minimum of a baccalaureate degree is required; graduate degree preferred.
  • 2 years of rehabilitation experience preferred.
