About the position

The Case Manager at CareStar, Inc. plays a crucial role in providing statewide case management services for individuals qualifying for the Rare and Expensive Case Management (REM) Program through the Maryland Department of Health. This position involves identifying, coordinating, and overseeing the delivery of services to enhance the health, safety, and well-being of participants, helping them achieve their goals towards maximum independence.

Responsibilities

• Identify, coordinate, and oversee the delivery of services for participants.
• Function as the team leader in developing and implementing consumer care plans.
• Complete home visits and monitor services in compliance with CareStar expectations.
• Conduct Person-Centered Planning Process and various assessments as per established guidelines.
• Complete incident reports and follow up using the state-approved process.
• Identify and report high-risk situations and take appropriate actions to mitigate health risks.
• Adhere to CareStar policies and maintain confidentiality regarding patient information.

Requirements

• Current/active license as a Licensed Social Worker, Licensed Independent Social Worker, or Registered Nurse.
• Bachelor's Degree in Nursing, Social Services, or a related field.
• Nationally Recognized Certification in Case Management within two years of employment.
• At least two years of experience in providing healthcare case management services to individuals with complex medical needs.

Nice-to-haves

• Experience in Long-Term Care Case Management.
• Familiarity with Person-Centered Planning Process.
