About the position

The Case Manager for the Stepping Up Program is responsible for providing comprehensive case management services to adults with serious mental illness and/or substance use disorders. This role focuses on preventing future hospitalizations and incarcerations by linking clients to community resources and ensuring continuity of behavioral health care. The position involves developing and implementing case management plans, maintaining records, and advocating for clients' needs within the community.

Responsibilities

  • Coordinates and provides case management services to recipients in the 'Stepping Up' program.
  • Completes case management needs assessments and develops a case management plan.
  • Implements the plan by providing case management services in conjunction with county jails and area hospital staff.
  • Advocates for recipients' needs with community providers.
  • Links recipients with community services and monitors their progress in the program.
  • Represents the needs of persons admitted into the 'Stepping Up' program and ensures quality services are provided.
  • Completes needs assessments as required and reviews needs on a regular basis.
  • Develops and implements case management service plans for recipients, consulting with referral sources and assisting with client goals.
  • Conducts periodic assessments and modifications of service plans, advocating on behalf of recipients.
  • Maintains recipient records in accordance with all applicable regulations and policies.
  • Treats recipients with care, dignity, and compassion, ensuring their privacy and confidentiality.
  • Performs other duties as assigned, including serving as a liaison and advocate for recipients with community agencies.

Requirements

  • Bachelor's degree in psychology, social work, human services, or a related field.
  • Valid driver's license with a clean motor vehicle record (MVR).
  • Ability to maintain automobile liability insurance.
  • Knowledge of psychiatric and substance use disorders in the adult population.
  • Ability to design, implement, and administer individualized behavioral services in regard to case management services.
  • Excellent verbal and written communication skills.
  • Ability to work independently and in a team environment.

Nice-to-haves

  • Prior case management experience in a mental health/substance use setting.
