About the position

The Case/Victim Service Manager is responsible for conducting risk assessments, tracking participant progress, and facilitating case closures. This role involves making referrals to partner organizations, maintaining relationships with local service providers, and collaborating on initiatives that serve shared populations. The manager will also coordinate care and treatment, assess treatment barriers, and connect youth and families to appropriate resources.

Responsibilities

  • Conduct in-depth risk assessment for participants, track progress, and execute case closures.
  • Make referrals to partner organizations or agencies that provide services such as mental health counseling, job training, medical care, housing assistance, legal aid, etc.
  • Track and record case information, complete forms, and produce reports.
  • Facilitate care and treatment via scheduling, resource sharing, and case coordination.
  • Assess and act accordingly in response to treatment barriers.
  • Connect youth and their families to other sports programs for youth unable to participate in YVITS sports programs.
  • Build and maintain relationships on a monthly basis with local service providers, government agencies, and other nonprofits to expand the network of available resources.
  • Collaborate on projects or initiatives that serve shared populations.

Requirements

  • Bachelor's degree in social work, psychology, sociology, human services, public health, or a related field.
  • Experience building relationships with service providers or collaborating with community-based organizations.
  • Three or more years of experience in case management or community outreach.
  • Experience with high-risk individuals and youth development.
  • Excellent interpersonal, verbal, and written communication skills.
  • Superb critical-thinking, organizational, and analytical skills.
  • Ability to work with all age groups and populations.
