About the position

The Case Worker - Conservatorship role involves providing ongoing case management for individuals deemed incapacitated by the Tribal Court. The position requires collaboration with various departments and agencies to ensure the well-being and proper care of clients under conservatorship. Responsibilities include managing personal affairs, coordinating services, and maintaining communication with families and legal entities.

Responsibilities

  • Provide ongoing case management for individuals determined incapacitated by the Tribal Court.
  • Assess for Vulnerable Adult/Conservatorship care needs.
  • Manage Conservatorship of the Person (COP) cases and ensure APS accuracy and compliance.
  • Coordinate activities and partnerships between MTSS, legal department, courts, and other resources.
  • Make decisions on behalf of clients and arrange for their personal and health care needs.
  • Maintain partnerships with fiduciary firms to protect the Conservatee's income and assets.
  • Manage court calendars and attend court hearings as needed.
  • Prepare accurate and timely court reports and coordinate case filings.
  • Assist in crisis interventions and conduct home studies when appropriate.
  • Engage and monitor services, including communication with outside providers and site visits.
  • Maintain case files and contact notes, updating case status as required.
  • Participate in on-call rotation duties and respond to hotline inquiries and emergencies.

Requirements

  • Bachelor's degree in Human Services, Social Work, Psychology, or related field.
  • Minimum of three years experience in social services, case management, and advocacy.
  • Valid California Driver's License and insurable on company policy.
  • Knowledge of legal proceedings, terminology, and court report writing.
  • Understanding of social service programs and appropriate forms used for each case.
  • Knowledge of Federal legislation affecting American Indian Tribes, particularly ICWA.
  • Awareness of trauma's impact on personal and social functioning.
  • Effective oral and written communication skills.

Nice-to-haves

  • At least three years experience working with Tribes or supporting Native Americans.
  • One year of conservatorship-related experience.
