Responsibilities

• Provide crisis prevention and management for individuals with IDD.
• Conduct assessments and service planning for clients enrolled in various Medicaid Waiver Programs.
• Coordinate access to medical, social, educational, and other services for clients.
• Advocate for clients and ensure consistent interactions with other agencies.
• Document and ensure the implementation of planned services and supports.

Requirements

• Bachelor's or advanced degree in a behavioral, social, or human service field.
• Prefer experience working with individuals with IDD.
• Must be able to work independently and as part of a team.
• Good organizational skills are necessary.

Nice-to-haves

• Qualified Intellectual Disabilities Professional (QIDP) certification preferred.
