About the position

The Caseworker PASRR Specialist position involves providing Pre-admission Assessment Screening and Resident Review (PASRR) services to individuals with mental illness. The role focuses on evaluating the need for nursing facility services, identifying alternative placements, and coordinating care with nursing facility staff. The specialist will also assist families and community providers with referrals and ensure that services are provided in a timely manner across multiple counties.

Responsibilities

  • Conduct screenings and evaluations to determine the need for nursing facility services for individuals with mental illness.
  • Identify alternative placement options and specialized services for clients.
  • Complete intakes with individuals identified as having positive PASRR.
  • Participate in interdisciplinary meetings with nursing facility staff to coordinate care and assess client needs.
  • Assist families and community providers with referrals to the PASRR program.
  • Monitor the TMHP LTC portal daily to ensure timely provision of requested services.
  • Act as the primary clinician for all PASRR clients across seven counties.

Requirements

  • Bachelor's Degree in a Behavioral, Social Science, Humanities, or Education field.
  • Experience working with individuals with mental illness.
  • Ability to work independently and as part of a team.
  • Good organizational skills.

Nice-to-haves

  • Experience in crisis management or on-call situations.
  • Familiarity with the Adult Needs and Strengths Assessment (ANSA) process.
