About the position

The Catastrophic Nurse Case Manager at Paradigm Corp is a home-based position responsible for managing medical cases related to work-related injuries. This role involves assessing, planning, coordinating, implementing, and evaluating care for injured or disabled individuals, primarily in the context of workers' compensation. The Case Manager collaborates with various stakeholders, including insurance carriers, medical providers, and employers, to ensure optimal outcomes for injured workers in the Jacksonville, FL area.

Responsibilities

• Manage medical case management for work-related injuries.
• Conduct assessments and develop care plans for injured/disabled individuals.
• Coordinate care with insurance carriers, medical providers, and employers.
• Monitor the progress of injured workers and report results back to employers and insurance carriers.
• Travel to various locations, including doctors' offices and hospitals, for case management purposes.

Requirements

• Current, unencumbered compact registered nurse (RN) license in FL.
• Experience in worker's compensation catastrophic case management.
• Critical care experience or experience in catastrophic injury/illness case management.
• Required certifications: CCM, CDMS, CRRN, COHN, or COHN-S.
• Strong organizational skills and ability to adhere to timelines.
• Effective communication skills (written, verbal, and interpersonal).
• Valid driver's license and good driving history.

Nice-to-haves

• Experience in clinical settings involving acquired brain injury, spinal cord injury, multiple trauma, or burns.
• Ability to work outside of traditional business hours if needed.
