About The Position

The Collaborative Drug Therapy Management (CDTM) Neurology Pharmacist plays a crucial role in enhancing patient care by collaborating with physicians and pharmacy teams. This position focuses on optimizing drug therapy, ensuring adherence, and managing disease states to achieve positive patient outcomes. The pharmacist will provide consulting services, perform medication reviews, and educate patients on their medication therapies, all while working within a structured CDTM protocol.

Requirements

  • Doctor of Pharmacy degree.
  • Minimum of 5 years of experience as a licensed pharmacist.
  • Current license and registration by the Massachusetts Board of Registration in Pharmacy.
  • Analytical skills to collect and analyze clinical information from diverse sources.
  • MA Controlled Substance License and DEA License must be obtained within 6 months of hire.

Nice To Haves

  • Previous pharmacist experience with neurology patients.
  • Previous pediatric pharmacist hospital experience.

Responsibilities

  • Interviewing patients to gather relevant health data, including medical and medication history.
  • Performing comprehensive medication reviews and making necessary modifications to achieve therapeutic outcomes.
  • Conducting routine patient monitoring functions related to drug therapy management.
  • Documenting all patient interactions, interventions, and progress notes consistently.
  • Collaborating with the care team to develop and promote adherence to patient care plans.
  • Communicating therapy changes to patients and caregivers clearly and concisely.
  • Performing follow-up calls to patients and caregivers regarding therapy.
  • Monitoring patient responses to therapy, focusing on safety and effectiveness.
  • Providing drug information to patients and healthcare professionals, adjusting communication based on the audience.
  • Educating patients on their medication therapies and assisting with medication access issues.
  • Establishing measurable patient outcomes and focusing on improving clinical effectiveness and patient safety.
