About The Position

The CDTM Pharmacist position at NYC Health + Hospitals involves providing comprehensive pharmaceutical care in an outpatient ambulatory setting. The role includes interviewing patients to gather relevant health data, assessing medication regimens, and making necessary modifications to achieve desired therapeutic outcomes. The pharmacist will also support patients in obtaining prescriptions, confirm insurance coverage, and collaborate with patient assistance programs to reduce financial barriers to therapy. Additionally, the pharmacist will serve as a resource for internal team members, oversee the training and development of clinical pharmacy residents or interns, and ensure compliance with all pharmacy licensing and regulatory requirements.

Requirements

• A valid New York State license and current registration to practice pharmacy.
• Certification to practice Collaborative Drug Therapy Management (CDTM) by the New York State Education Department (NYSED).

Responsibilities

• Interview patients to gather relevant health data including medical and medication history.
• Screen for drug allergies and interactions, noting contraindicated drugs for specific disease states.
• Assess medication regimens and modify them to achieve desired therapeutic outcomes.
• Assist patients or caregivers in obtaining prescriptions and confirm insurance coverage and formulary status.
• Collaborate with patient assistance programs and finance teams to reduce financial barriers to therapy.
• Serve as an operational resource for internal team members to resolve barriers to therapy.
• Oversee the training, development, and evaluation of clinical pharmacy residents or interns.
• Maintain all pharmacy licensing and regulatory requirements.
