About the position

The Certified Home Health Aide (CHHA) position at the Karen Ann Quinlan Memorial Foundation involves providing essential personal care and support to patients in their homes. The role focuses on assisting with daily living activities, monitoring vital signs, and ensuring the overall well-being of patients, particularly those who are bed-bound or require special care. This per diem position requires flexibility, as shifts may include evenings and weekends.

Responsibilities

  • Assist patients with personal and oral hygiene, including bathing and skin care.
  • Accurately take and record vital signs.
  • Safely assist patients with transfers to bed, chair, and commode.
  • Assist bed-bound patients with range of motion exercises and positioning.
  • Perform accurate recording of intake and output (I&O).
  • Follow bowel/bladder training programs and provide incontinence care.
  • Perform catheter care according to policy and procedure.
  • Apply colostomy bags, anti-embolic stockings, braces, and simple dressings as instructed by the RN.
  • Administer tepid sitz baths and assist patients in self-administering prescribed medication.
  • Demonstrate safe use of hoyer lift as instructed by the RN.
  • Prepare meals for patients and assist with feeding according to dietary instructions.
  • Observe and report changes in patients' physical and emotional status.
  • Assist with post-mortem care as instructed by the RN.
  • Complete documentation accurately and submit via Celltrack.
  • Change occupied or unoccupied beds safely.
  • Maintain a neat and clean patient environment.
  • Incorporate consideration of patients' physiological and developmental age into care delivery.
  • Participate in required in-service education programs and meetings.
  • Adhere to attendance standards and notify in advance for absences or tardiness.
  • Comply with dress code and maintain a professional appearance.

Requirements

  • Valid Certified Home Health Aide certification.
  • Ability to assist with personal care and hygiene tasks.
  • Experience in monitoring vital signs and patient care.
  • Knowledge of safe patient transfer techniques.
  • Ability to follow care plans and instructions from RNs.

Nice-to-haves

  • Experience working with elderly or disabled patients.
  • Knowledge of dietary restrictions and meal preparation for patients.
  • Familiarity with documentation processes in home health care.
