As a Certified Nursing Assistant, you will be a vital member of the interdisciplinary team, working under the supervision of a Registered Nurse to provide essential services that meet the personal needs of patients and promote their comfort. In this role, you will be responsible for observing patients, documenting care provided, and reporting any significant changes in their condition to the Registered Nurse Case Manager. Your assignments will be structured to ensure quality, continuity, and safety in patient care. You will perform a variety of personal care tasks, including assisting with bathing, dressing, feeding, and maintaining a clean living environment for the patient. Additionally, you will be expected to assist with mobility and exercise as directed by healthcare professionals, and provide respite for patients and their families when necessary. This position requires adherence to the organization's documentation standards and a commitment to maintaining professional conduct at all times. The Certified Nursing Assistant role is crucial in delivering compassionate care to patients in their homes, ensuring their comfort and safety while respecting their personal needs and environment. You will also participate in quality assessment and performance improvement activities, contributing to the overall enhancement of care provided by the organization.

Responsibilities

  • Perform personal care needs after given a patient's assignment and attend to requests promptly.
  • Report to the Registered Nurse Case Manager if unable to perform a certain task.
  • Meet safety needs of the patient and use equipment safely and properly (foot stools, side rails, O2, etc.).
  • Give personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linen as assigned.
  • Assist in dressing and undressing patients as assigned.
  • Assist in feeding the patient as assigned.
  • Take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered with appropriate skills competency.
  • Provide proper care and observation of the patient's skin to prevent breakdown of tissue over bony prominence.
  • Report on the patient's condition and significant changes to the Registered Nurse Case Manager.
  • Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
  • Offer and assist with bedpans and urinals.
  • Provide assistance with light laundry needs as assigned.
  • Perform range of motion and other simple procedures as an extensional therapy service as ordered with appropriate skills competency.
  • Provide respite for patient's/family/caregiver when on-site, as appropriate.
  • Keep the patient's immediate living space tidy, as assigned.
  • Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
  • Participate in quality assessment performance improvement teams and activities.
  • Be respectful of the patient and family/caregiver environment and the patient's personal needs.
  • Participate in on-call as assigned.

Requirements

  • Successfully completed a state-established or other training program that meets the requirements of S 484.36(a) and a competency evaluation program, or state licensure program that meets the requirements of S 484.36(b).
  • One year of Certified Nursing Assistant experience required.
  • One year of home health/hospice experience preferred.
  • At least 18 years of age.
  • Ability to read and follow written instructions and document care given.
  • Understanding of home health practices.
  • Self-directing with the ability to work with little direct supervision.
  • Ability to provide a calm manner when in a patient's home.
  • Empathy for the needs of the ill, injured, frail, and impaired.
  • Current CPR certification.
  • Demonstrates tact, patience, and good personal hygiene.
  • Licensed driver with an automobile that is insured in accordance with organization requirements and is in good working order.

Nice-to-haves

  • Experience in a home health or hospice setting is preferred.
