Jet Health, Inc. is seeking a dedicated Certified Nursing Assistant (CNA) to join our team in Grand Junction, CO. This position involves performing home health visits for patients under the supervision of a Registered Nurse. In this role, the Certified Nursing Assistant will be a vital member of the interdisciplinary team, responsible for meeting the personal needs of patients and promoting their comfort. The CNA will observe patients, report observations, and document care provided. Assignments will be made to ensure quality, continuity, and safety in patient care. The position requires a compassionate individual who can effectively assist patients with their daily activities while maintaining a respectful and supportive environment. The Certified Nursing Assistant will perform a variety of personal care tasks, including bathing, dressing, feeding, and assisting with mobility. Additionally, the CNA will monitor vital signs, provide skin care, and assist with light housekeeping tasks as needed. Participation in quality assessment and performance improvement activities is also expected, along with adherence to the organization's documentation standards and professional conduct. This position may require on-call duties as assigned, and the CNA must meet the training requirements set by the state.

Responsibilities

  • Perform personal care needs after receiving patient assignments and attend to requests promptly.
  • Meet safety needs of patients and use equipment safely and properly (e.g., foot stools, side rails, O2).
  • Provide personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linens as assigned.
  • Assist in dressing and undressing patients as assigned.
  • Assist in feeding patients as assigned.
  • Take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered, demonstrating appropriate skills competency.
  • Provide proper care and observation of patients' skin to prevent breakdown of tissue over bony prominence.
  • Report on patients' conditions and significant changes to the Registered Nurse Case Manager.
  • Assist in ambulation and exercise as instructed by the hospice nurse or therapist.
  • Offer and assist with bedpans and urinals.
  • Provide assistance with light laundry needs as assigned.
  • Perform range of motion and other simple procedures as an extensional therapy service as ordered, demonstrating appropriate skills competency.
  • Provide respite for patients/family/caregiver when on-site, as appropriate.
  • Keep patients' immediate living space tidy, as assigned.
  • Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
  • Participate in quality assessment performance improvement teams and activities.
  • Respect the patient and family/caregiver environment and the patient's personal needs.
  • Participate in on-call duties as assigned.

Requirements

  • Must have successfully completed a state-established or other training program that meets the requirements of S 484.36(a) and a competency evaluation program, or state licensure program that meets the requirements of S 484.36(b).
  • One year of Certified Nursing Assistant experience required.
  • One year of home health/hospice experience preferred.
  • At least 18 years of age.
  • Ability to read and follow written instructions and document care given.
  • Understanding of home health practices.
  • Self-directing with the ability to work with little direct supervision.
  • Ability to provide a calm manner when in a patient's home.
  • Empathy for the needs of the ill, injured, frail, and impaired.
  • Possess and maintain current CPR certification.
  • Demonstrates tact, patience, and good personal hygiene.
  • Licensed driver with an automobile that is insured in accordance with organization requirements and is in good working order.

Nice-to-haves

  • Experience in a home health or hospice setting is preferred.
