Responsibilities
- Provide in-home personal care and designated health-related services to maintain patients' physical and emotional well-being.
- Follow the written plan of care and adhere to Medicare/Medicaid regulations and agency policies.
- Assist patients with activities of daily living (ADLs) such as mobility transfer, grooming, bathing, dressing, eating, and toileting.
- Perform incidental household services essential to the patient's health care at home to prevent or postpone institutionalization.
- Complete a clinical note for each visit within required timeframes to be incorporated into the patient's record.
- Provide care according to the aide plan of care/assignment sheet and in accordance with State Practice Act or regulations.
Requirements
- Successful completion of a competency evaluation.
- Current CPR certification required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Ability to read, write, and carry out directions promptly and accurately.
