About the position

The Certified Nutritionist will serve as a subject matter expert in nutrition, leading the Diabetes Prevention Program at the Siletz Community Health Clinic. This role involves providing individual nutrition assessments, counseling, and education, as well as coordinating care with other health professionals to support the nutritional needs of Siletz Tribal members and the community.

Responsibilities

  • Complete individual nutrition assessments and therapy for patients identified as diabetic, pre-diabetic, or at-risk.
  • Assist with outreach content around nutrition and support the Diabetes and Community Health departments.
  • Develop individual nutrition plans considering medical needs, laboratory results, cultural preferences, and personal goals.
  • Coordinate care between patients and clinic medical providers.
  • Compile nutritional resources for staff and assist in patient care plans.
  • Teach group education sessions related to nutrition and oversee training by other staff.
  • Develop nutrition lesson plans and educational materials.

Requirements

  • Certified Nutritionist with a license to work in Oregon and Washington, or eligible to apply for one.
  • Minimum 2 years of experience in a clinical setting, preferably with diabetes patients.
  • Knowledge of human nutrition and its relationship to health and disease.
  • Proficient in Microsoft Office (Excel, Word, PowerPoint) and experience with NextGen EMR preferred.
  • Good oral and written communication skills, with strengths in teamwork and customer service.

Nice-to-haves

  • Certified Diabetes Care and Education Specialist (CDCES) preferred or interest in obtaining certification.
  • Experience working with the Native American/Alaska Native community.
