About the position

The Certified Peer Specialist at The Bridge Inc. plays a crucial role in supporting adults with substance use disorders and/or mental health diagnoses. This position involves engaging with individuals in various settings, conducting outreach, and facilitating individual sessions to promote recovery and independent community living. The specialist will work closely with clients to navigate community resources, manage care plans, and foster relationships that aid in their transition back into the community.

Responsibilities

  • Engage with NYC homeless individuals at known hang-outs or during hospital admissions.
  • Conduct outreach and training on Narcan and Fentanyl test strips for high-risk individuals.
  • Facilitate individual sessions focusing on prevention and independent living.
  • Assist participants with family and social relationships, stress management, and daily living activities.
  • Work with housing providers to resolve clinical issues affecting housing stability.
  • Foster community relationships to connect clients with appropriate services.
  • Navigate appointments and provide travel training for community care re-engagement.
  • Conduct comprehensive psychosocial assessments to determine participants' needs.
  • Monitor and evaluate participants' progress towards care plan goals.
  • Attend team meetings and supervisory sessions.

Requirements

  • Bachelor's Degree or equivalent experience in case management (4 years may substitute for degree).
  • Minimum three years of experience providing services to adults with serious mental illness preferred.
  • Experience in case management within a social service agency, preferably in behavioral health.
  • Knowledge of homeless resources, NYC shelter systems, and MTA transit systems.
  • Familiarity with counseling principles for mental illness and substance use disorders.
  • Ability to develop and modify treatment interventions based on individual needs.
  • Proficiency in computer skills and documentation.
  • Experience with electronic health records, preferably AWARDS by Footholds.
  • Spanish speaking is highly desirable.

Nice-to-haves

  • Experience working with homeless or precariously housed populations.
  • Knowledge of treatment, rehabilitation, and community support programs.
