Responsibilities

• Maintain a caseload of Intensive Case Management clients and assist them in various recovery life skills consistent with their needs and preferences.
• Provide guidance on personal care and independent living skills.
• Promote personal health and safety awareness.
• Assist clients with money management.
• Coordinate transportation for patients to necessary community services.
• Attend community meetings to market the program and develop community resources.
• Advocate to ensure patients have access to and receive services from community-based programs.
• Monitor client's progress in completing recovery goals.

Requirements

• High school diploma or GED required.
• At least 1 year of experience in medical or mental health or a related area required.
• CCAR (Connecticut Community for Addiction Recovery) Certification required within 3 months of hire.
• Experiential or actual knowledge of Medication Assisted Treatment (MAT).
