About the position
Holisticare Hospice is seeking a Part-Time Chaplain to provide spiritual care to patients and families in the Pleasantville, NJ area. This role is integral to the Interdisciplinary Team (IDT), focusing on delivering spiritual counseling and emotional support to both active and bereaved individuals. The Chaplain will implement and coordinate spiritual activities in alignment with hospice policies, contributing to the overall mission of providing quality end-of-life care.
Responsibilities
- Assess the spiritual status of patients and families/caregivers related to the patient's terminal illness and environment.
- Communicate findings to the RN Case Manager and interdisciplinary group.
- Provide direct spiritual care for all faiths/non-faiths to patients and families/caregivers in a holistic manner as requested.
- Maintain regular communication with the Program Director to review clients and manage spiritual care needs.
- Initiate communication with hospice staff members and other facilities to coordinate optimal care and resource use for patient/family members.
- Monitor and maintain documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness.
Requirements
- Graduate of accredited seminary or school of theology, or appropriate certification in pastoral ministry if required by state.
- Minimum two units of Clinical Pastoral Education (four units strongly preferred).
- Minimum two (2) years experience as a healthcare pastor/chaplain.
- One (1) year of hospice experience preferred but not required.
- Ability to handle stress associated with ministering to patients/residents and their loved ones who experience pain, suffering, death, and grief.
- Computer proficiency in typing and various programs, including background in EMR.
- Current CPR certification if required by state.
