About the position

The Chaplain Part Time position involves providing in-home spiritual care to patients and their families within the hospice program. The role emphasizes the importance of emotional and spiritual support during illness and loss, ensuring that the spiritual needs of patients from diverse backgrounds are met. The chaplain will work closely with the interdisciplinary team and community clergy to facilitate comprehensive spiritual care.

Responsibilities

  • Contact patients/caregivers within five days of admission to conduct an initial spiritual assessment.
  • Complete and document ongoing assessments, interventions, and outcomes in the Electronic Medical Record.
  • Assist the Interdisciplinary Team in understanding significant religious issues related to terminal illness and bereavement.
  • Coordinate and organize home visits efficiently.
  • Work with community ministers to support diverse spiritual needs of patients.
  • Communicate with and support the patient's/caregiver's clergy as appropriate.
  • Provide consultation and education on spiritual/religious concerns affecting end-of-life care.
  • Assist caregivers with funeral arrangements and services as appropriate.
  • Provide counseling to the Interdisciplinary team and volunteers as needed.
  • Utilize automated devices for documentation in the electronic medical record.
  • Be available for on-call rotation for after-hours visits during spiritual crises or patient deaths.
  • Assist with bereavement calls to caregivers following a patient's death.

Requirements

  • A graduate degree from an accredited theological school or seminary with emphasis in religion, divinity, pastoral counseling, or related field.
  • At least 1 year of chaplaincy experience in a healthcare setting preferred.
  • Comfort in an interfaith setting and ability to work with individuals from diverse backgrounds.

Nice-to-haves

  • M.Div. preferred.
