About the position
Amedisys is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) and Certified Nursing Assistant (CNA) to join our hospice team in Bloomfield, New Jersey. This position is PRN (as needed) and specifically requires coverage on weekends only. The ideal candidate will be responsible for providing personal care services to patients in their homes, ensuring their comfort and dignity while adhering to safety principles and proper body mechanics. The role involves assisting with daily living activities, including ambulation, transferring clients, and preparing nutritious meals. Additionally, the aide will perform homemaking tasks, observe and document any changes in the patient's condition, and follow specific care instructions provided by registered nurses. Responsibilities
- Provides personal care services to patients.
- Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
- Plans and prepares nutritious meals, assists with feeding, as assigned.
- Performs homemaking and other environmental services as assigned.
- Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse.
- Follows specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care.
- Assists patients with prescribed exercises as instructed by nurse or therapist.
- Performs and records simple urine tests for sugar, acetone, and albumin.
- Other duties as assigned.
Requirements
- High school diploma or equivalent.
- Current state certification (or registration) as a home health aide (CHHA) as per state certification/registration requirements.
- Current state certification (or registration) as a nurse assistant (CNA) as per state certification/registration requirements.
- CHHA experience within the last two years. If it has been more than two years, you must complete another training and competency program before providing services.
- Must be able to successfully pass the competency program of the care center.
- Current CPR certification.
