About the position

The Child and Adolescent Continuity of Care Liaison (C&A COC Liaison) plays a crucial role in overseeing the care of youth within various treatment facilities, including state hospitals and residential treatment centers. This position is responsible for case management, connecting youth and their families with appropriate services, and ensuring continuity of care through effective discharge planning and coordination of outpatient services. The C&A COC Liaison works under moderate supervision and is expected to adhere to the Center's guiding principles while actively participating in crisis intervention and support for individuals at risk.

Responsibilities

  • Act as a liaison between the Center and state psychiatric hospitals, youth residential treatment centers, and other community providers.
  • Document all interactions in the individual's electronic health record.
  • Ensure all required discharge paperwork is obtained from state psychiatric hospitals and residential treatment centers.
  • Facilitate placement for youth into residential treatment centers.
  • Communicate weekly with youth or parent/guardian during the application process to residential treatment centers.
  • Maintain a list of individuals currently in each state hospital for follow-up and aftercare.
  • Participate in discharge planning for individuals in local state hospitals.
  • Present in person and/or virtually to state hospitals and residential treatment centers as needed.
  • Complete aftercare appointments for individuals discharged from inpatient hospitalization or residential treatment centers within 7 days of discharge.
  • Coordinate meetings and communication for community partners involved in Community Resource Coordination Group (CRCG).
  • Assist families in completing applications to attend CRCG and inform community partners of applications and referrals.
  • Attend community events as assigned by the supervisor.
  • Complete 24-hour follow-up for individuals in local psychiatric hospitals via phone or face-to-face as necessary.

Requirements

  • Graduation from an accredited four-year college or university with a Bachelor's degree in a human services field.
  • One year of related experience working in the mental health field.
  • Experience working with individuals with intensive needs preferred.
  • Knowledge of community resources and case management delivery systems.
  • Ability to assess individual needs and coordinate services effectively.
  • Ability to maintain a calm demeanor during crisis situations.
  • Understanding and practice of Trauma Informed Care principles.

Nice-to-haves

  • Experience participating as a member of an interdisciplinary team in developing and implementing individualized program plans for mental health consumers.
