About the position
The Children & Family Home/Community-Based Case Manager at Swanson Center is responsible for providing comprehensive case management services to children and adolescents. This role involves working closely with clients and their families to develop treatment plans, conduct assessments, and coordinate care with various agencies. The position emphasizes a commitment to improving the lives of clients through effective communication, advocacy, and crisis intervention.
Responsibilities
- Perform case management duties including intake, treatment planning, and follow-up care.
- Visit clients in the community and/or their homes to provide necessary services.
- Schedule therapy and doctor appointments for clients and assist with personal affairs.
- Link clients to other services and monitor their progress.
- Create and update treatment plans and ensure continuity of care for assigned clients.
- Advocate for clients to improve their general well-being and assist with emergency procedures.
- Develop and maintain linkages with client families and outside agencies.
- Document services and activities in a professional manner and submit required records accurately and timely.
- Attend and participate in designated staff meetings and quality assurance systems.
Requirements
- Bachelor's degree in Social Work, Psychology, or a health-related field.
- Experience working with children and their families is preferred.
- Valid driver's license and a good driving record.
- Registered four-wheeled vehicle for client transportation.
