About the position

Swanson Center is seeking a dedicated Children & Family Home/Community-Based Case Manager to join our compassionate team in Michigan City, IN. This position is integral to our mission of improving lives through comprehensive mental health services. As a member of a child and adolescent team, the Case Manager will perform a variety of responsible activities, including case management duties in areas such as intake, treatment planning, individual and family skills training, group skills development, discharge planning, and follow-up care. The Case Manager will be responsible for managing cases assigned or referred for treatment by the Department of Child Services (DCS). This role also involves consultation and coordination with various agencies, including schools, juvenile probation, courts, and other child-caring services. The ideal candidate will possess a Bachelor's degree in Social Work, Psychology, or a related health field, along with training and experience in working with children and families. The Case Manager will be expected to visit clients in their homes and communities, demonstrating professionalism and commitment to the Center's mission. Responsibilities include scheduling therapy and doctor appointments, performing standard case management tasks, monitoring client progress, and ensuring the timely submission of all required documentation. The Case Manager will advocate for clients to improve their well-being and assist with emergency procedures and court referrals as necessary. This position requires a valid driver's license, a good driving record, and a registered vehicle for client transportation.

Responsibilities

  • Perform case management duties including intake, treatment planning, and follow-up care.
  • Visit clients in the community and/or in their homes.
  • Schedule therapy and doctor appointments for clients.
  • Link clients to other services and monitor their progress.
  • Create and update treatment plans and discharge planning.
  • Advocate for clients to improve their general well-being.
  • Assist with emergency detention procedures and court referral proceedings.
  • Maintain accountability for assigned clients' progress and continuity of care.
  • Ensure timely and accurate submission of all required records of client care and staff activity.
  • Participate in designated staff meetings and quality assurance systems.

Requirements

  • Bachelor's degree in Social Work, Psychology, or a health-related field.
  • Training, education, and experience in working with children and families preferred.
  • Valid driver's license and proof of insurance.
  • Registered four-wheeled vehicle with a good driving record.
  • Ability to document services and activities professionally.
  • Effective communication skills with families and professionals.
  • Ability to identify and report ethical violations and crises.
  • Analytical skills to draw relevant conclusions from information.
