The Children's BHH Care Coordinator/Case Manager position at Spurwink Services involves working with children and families within the Lewiston area. Responsibilities

• Coordinate and collaborate with providers working with each child to ensure their needs are addressed.
• Complete intakes for new clients and conduct psychosocial assessments.
• Manage weekly documentation needs and make referrals as necessary.
• Develop initial and 90-day treatment plans for clients.
• Maintain a minimum of monthly contact with each family to provide ongoing support and collaboration with providers.

Requirements

• Bachelor's degree in a related field is required.
• Strong organizational and communication skills are essential.
• Proficient computer skills to manage multiple systems are necessary.
• Must maintain a current driver's license and a clean driving record.

Nice-to-haves

• Experience in a clinical or case management role is preferred.
• Familiarity with mental health services for children and families.
