About the position

The Children's Long-Term Support Case Manager is responsible for managing a caseload of children and youth with various medical, developmental, emotional, and mental health needs. This role involves conducting assessments, coordinating services, and collaborating with families and service providers to ensure the delivery of appropriate support. The position requires strong organizational skills and the ability to maintain effective working relationships with clients and colleagues.

Responsibilities

  • Accepts requests and referrals for agency services
  • Explains scope of services and consumer rights and responsibilities
  • Conducts strength-based assessments through interviews, home visits, and collecting collateral information
  • Determines the range and type of needed services in collaboration with parents/caregivers and children/youth
  • Coordinates appropriate services based on assessed needs
  • Assesses ongoing needs to update service plans
  • Prepares correspondence, reports, and records as necessary
  • Coordinates and facilitates team meetings to collaboratively work with families
  • Monitors ongoing delivery of services to consumers
  • Participates in committees and task forces as assigned
  • Maintains documentation that meets professional practice and program standards
  • May be required to work overtime hours or shifts under the direction of the Social Work Manager

Requirements

  • Bachelor's degree in social work or closely related human services field
  • Three years' experience with individuals or families with mental health, substance abuse, and/or abuse and neglect issues preferred
  • Knowledge of child development and childhood disorders
  • Good written and oral communication skills; tact and courtesy; thoroughness and dependability
  • Knowledge of the functions, procedures, organization, policies, and regulations governing the administration of the Children's Long-Term Support Program
  • Ability to provide services to children and youth with special needs and their families
  • Ability to maintain confidentiality in dealing with all pertinent information
  • Ability to establish and maintain effective working relationships with peers and supervisors
  • Valid motor vehicle operator's license and/or available means of transportation required

Nice-to-haves

  • Social Work Certification preferred, but not required
