About the position

The CHOICES Case Manager at East Tennessee Human Resource Agency, Inc. is responsible for conducting in-home assessments for older clients and individuals with disabilities to determine eligibility for the CHOICES Home and Community Based Program. This role involves working closely with clients in Knox and surrounding counties, ensuring they receive the necessary support and documentation for TennCare applications. The position emphasizes flexibility, work-life balance, and offers a range of benefits.

Responsibilities

• Conduct in-home assessments for older clients and persons with disabilities for possible enrollment in the CHOICES Home and Community Based Program.
• Complete the TennCare Pre-Admission Evaluation and TennCare Financial Application while in the home.
• Gather supporting documentation to support the TennCare Applications.

Requirements

• Graduation from an accredited four-year college with a LPN, RN, Licensure in Social Work preferred or closely related field such as Human Services.
• Good organizational, interpersonal skills, and decision-making skills.
• Any equivalent combination of education and/or experience will be considered.
• Valid TN driver license.
