About the position
The Client Care Coordinator/Transportation role at Fourth & Hope is designed to support clients in their journey towards self-sufficiency and permanent housing. This position involves coordinating the basic needs and welfare of program participants, assisting them in developing essential living skills, and providing trauma-informed support. The role emphasizes community integration and transportation assistance, ensuring that clients receive the necessary resources and referrals to improve their quality of life.
Responsibilities
- Coordinate the basic needs, health, and welfare of program participants.
- Assist participants in developing healthy basic living skills.
- Provide engagement, basic assessment screenings, and resource and referral information related to education, employment, housing, and healthcare.
- Assist with community integration and transportation needs.
- Monitor and document the self-administration of medications and maintain accuracy of participant medication charts.
- Assist residents with arranging appointments and transportation.
- Collaborate with the case management team to meet residents' personal care needs.
- Maintain accurate and up-to-date records and ensure confidentiality.
- Participate in staff meetings and complete required training.
- Monitor ongoing health and safety standards.
Requirements
- High school diploma or GED.
- CDL B license preferred but not required.
- Knowledge of leadership principles and practices.
- Ability to communicate clearly and concisely, both orally and in writing.
- Experience with data entry systems and record keeping.
- Ability to work with diverse populations and cultural backgrounds.
