About the position

The Client Care Coordinator at Jenkins Home Care is responsible for ensuring that clients receive the highest quality of care by developing service plans tailored to their needs. This role involves direct interaction with clients and their families, as well as maintaining accurate records of care and quality assurance visits.

Responsibilities

  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
  • Plan and execute a schedule that ensures each client has, at minimum, one quality assurance visit per quarter.
  • Create and maintain client and responsible party records, documenting all quality assurance meetings.

Requirements

  • Must have worked for a home care agency—home care industry experience is a must.
  • Related business experience or an equivalent combination of education and work experience may be considered.
  • Must possess a valid driver's license.
