About The Position

The Client Care Coordinator at Home Instead is responsible for developing and maintaining positive relationships with clients and their families, ensuring quality care through regular visits and effective communication. This role involves coordinating care services, conducting consultations, and providing community education on senior care issues. The position requires a proactive approach to managing client needs and supporting care professionals in their roles.

Requirements

• 3+ years of experience in the Senior Care or Health Care Industry.
• Clinical Background.
• Ability to work independently in a field position.
• Excellent organizational skills and problem-solving skills.
• Strong customer service skills.
• A warm, friendly, compassionate demeanor with elderly clients.

Responsibilities

• Develop and maintain positive relationships with clients and their families through quality assurance visits, phone calls, and special events.
• Plan and execute a schedule that ensures each client has at least one quality assurance visit per month.
• Work with Recruiter to create, improve, and execute care professional's (CP) orientations and trainings.
• Meet with potential clients and family members to discuss their needs and provide solutions in the form of a Care Consultation.
• Communicate effectively with clients and family members, as well as other office staff and CPs to coordinate various aspects of a client's care.
• Monitor, mediate, and log both client and CP activity and follow up on CP's assignments and client service.
• Conduct client and CP introductions as scheduled.
• Provide community education regarding senior care issues and resources.
