About the position

The Client Care Field Representative plays a vital role in the home care services team, focusing on client intake, onboarding, and ensuring high-quality care delivery. This position requires strong customer service skills and offers opportunities for advancement into managerial roles.

Responsibilities

  • Conduct comprehensive client intakes and assessments
  • Develop and maintain relationships with new and existing referral sources
  • Vet and develop leads, performing initial in-home assessments
  • Sign up new clients and create individualized care plans
  • Match caregivers with clients, introducing them and reviewing care plans
  • Coordinate and schedule care services
  • Monitor and evaluate the quality of care provided
  • Manage client relationships and address concerns or feedback
  • Liaise between clients, families, caregivers, and healthcare professionals
  • Maintain accurate and up-to-date client records
  • Ensure compliance with state and federal regulations
  • Participate in marketing our company and services to the community
  • Take part in the on-call rotation (typically one week per month)
  • Support other operational departments as needed
  • Provide general office support and problem-solving

Requirements

  • 1-3+ years of experience in home care client services or related healthcare field
  • Experience with in-home client assessment and care plan development
  • Strong lead generation and sales skills for client intake
  • Excellent interpersonal and communication skills
  • Outstanding organizational and time management abilities
  • Proficiency in Microsoft Office and healthcare management software
  • Willingness to drive in the King/Snohomish/Pierce County area, with valid Washington State driver's license, reliable transportation, and clean driving record
  • Ability to pass a criminal background check

Nice-to-haves

  • Previous caregiving experience (HCA and/or CNA)
  • Private in-home care administrative experience
