About the position

The Client Relations Specialist at Golden Heart is responsible for matching clients with caregivers, supervising Host Home Providers, and ensuring client satisfaction through effective communication and care management. This role involves conducting wellness checks, managing care plans, and maintaining relationships with case managers and care coordinators to facilitate client onboarding and service delivery.

Responsibilities

  • Conduct wellness checks with clients to ensure care plans are satisfactory and make necessary updates.
  • Assist clients in securing caregiver hours to meet their service requirements.
  • Ensure new clients have the necessary items and resources for a healthy living environment.
  • Review referrals and assess new potential clients for program fit.
  • Secure sensitive information in compliance with HIPAA regulations.
  • Assist with finding coverage and working shifts when needed.
  • Conduct intakes with new veterans and seniors.
  • Check documentation and PDI's of host homes to ensure quality care.
  • Provide reports and updates on client placements from wellness checks.
  • Maintain open communication with team members regarding client onboarding tasks.
  • Foster healthy relationships with case managers and care coordinators for future referrals.
  • Keep records of completed tasks and update office software.
  • Utilize various communication methods effectively.

Requirements

  • 3 years or more of experience working with individuals with intellectual disabilities, mental health diagnoses, brain injury, autism spectrum, and other disabilities requiring assistance.
  • Associates or Bachelor's degree in Human Services or a related field.
