About the position

The Service Coordinator role at Easterseals PORT Health involves supervising developmental disabilities services for individuals enrolled in Empower programs. This full-time position focuses on ensuring compliance with state and federal standards while supporting the personal outcomes of individuals and families. The coordinator will manage enrollment documentation, oversee service referrals, and collaborate with various entities to provide tailored support to participants.

Responsibilities

• Supervise developmental disabilities services for individuals in Empower programs.
• Ensure compliance with state, federal, and agency standards for assigned services.
• Complete required enrollment documents for EHR and Provider Direct.
• Support management by ensuring necessary consents and authorizations are in place before services begin.
• Ensure services provided are within authorized limits.
• Complete the referral process correctly and efficiently, including person-centered plans and short-range goals.
• Report on progress towards goals to program supervisors, staff, and families quarterly.
• Collaborate with MCO and other entities to ensure supports are based on individual desires and needs.

Requirements

• Bachelor's degree from an accredited college or university.
• Minimum of two years' experience working with individuals with Intellectual and/or Developmental Disabilities.
• Valid, unrestricted state appropriate driver's license and personal vehicle insurance.
• Satisfactory criminal background check and acceptable motor vehicle record.
• Knowledge of Microsoft Word, Microsoft Excel, Zoom, and Microsoft Teams.
• Experience with EHR is a plus.
