About the position

The RN Client Success Manager at TheKey is responsible for ensuring the delivery of consistent, quality care to clients, focusing on their physical, emotional, and social well-being. This role involves maintaining accurate client care plans, conducting assessments, and building strong relationships with clients and their support systems. The position is integral to client satisfaction and retention, requiring collaboration with internal teams and adherence to state regulations.

Responsibilities

• Manage the day-to-day client relationship and delivery of quality care.
• Identify and focus on a holistic approach to client care with a focus on quality of life.
• Conduct in-person nursing initial and reassessments and quality assurance visits.
• Utilize interpersonal relationships and communication skills to build rapport with clients.
• Ensure that TheKey care team is up-to-date with all relevant information and care plans.
• Collaborate and communicate with internal and external customers regularly.
• Consult with clients regarding their specific needs and preferences.
• Act as a subject matter expert for quality control and assurance matters.
• Ensure compliance with state regulations and documentation requirements.
• Conduct thorough initial assessments for new clients.
• Manage caregiver introductions and recommend additional training as needed.
• Handle escalations and complaints from clients and caregivers.
• Review and adjust client rates based on changing care needs and satisfaction.
• Support on-call and after-hours needs as required.

Requirements

• Current Registered Nurse license in the state of employment.
• 5 years experience in health care, elder care, social work, or related industry preferred.
• Excellent customer service and conflict resolution skills.
• Computer proficiency and ability to document accurate notes.
• Current driver's license and proof of insurance.

Nice-to-haves

• Bachelor of Science in Nursing (BSN) preferred.
• Experience in quality assurance and quality control.
