About the position
The Clinic Care Coordinator serves as an advocate, educator, and coordinator of services for at-risk patients within the Cardiology department. This role involves working closely with a multidisciplinary team to ensure comprehensive care for patients undergoing various cardiovascular procedures and treatments.
Responsibilities
- Serve as an advocate for at-risk patients.
- Coordinate services and care for patients in the cardiology department.
- Educate patients about their health conditions and treatment options.
- Assist in managing patient care and follow-up appointments.
- Collaborate with healthcare providers to ensure continuity of care.
- Utilize electronic medical information systems for patient management.
Requirements
- High school diploma or equivalent required.
- Bachelor's degree in a health-related field and one (1) year experience in a healthcare setting, or an Associate degree with a Registered or Certified Medical Assistant and two (2) years' experience, or three (3) years experience in a healthcare setting with two (2) years professional customer service experience.
- Healthcare Provider Level BLS required within 90 days of hire.
- Certification or Registration as a Medical Assistant preferred.
Nice-to-haves
- Experience in outpatient, ambulatory care, and/or community healthcare.

- Experience in a primary care related setting.
- Experience with chronic conditions.
- Bilingual skills preferred.
