About The Position
The Clinic Care Coordinator serves as an advocate, educator, and coordinator of services for at-risk patients within the Samaritan InterCommunity Health Clinic. This role focuses on promoting equitable and culturally humble care, aligning health plans with clinical teams to enhance access, improve quality outcomes, reduce costs, and facilitate care coordination for patients.
