About the position

The Clinic Care Coordinator serves as an advocate, educator, and coordinator of services for at-risk patients within the Samaritan InterCommunity Health Clinic. This role focuses on promoting equitable and culturally humble care, aligning health plans with clinical teams to enhance access, improve quality outcomes, reduce costs, and facilitate care coordination for patients.

Responsibilities

• Serve as an advocate and educator for at-risk patients.
• Coordinate services to improve patient care and access.
• Work collaboratively with a multi-disciplinary team.
• Assist in managing patient care for diverse populations, including pediatrics and geriatrics.
• Maintain confidentiality of patient information in compliance with HIPAA regulations.
• Organize and prioritize work to meet required time frames.

Requirements

• High school diploma or equivalent required.
• One of the following: Bachelor's degree in a health-related field with one year of experience in a healthcare setting; Associate degree or completion of a technical program with Registered or Certified Medical Assistant and two years of experience; or three years of experience in a healthcare setting with two years of professional customer service experience.
• Healthcare Provider Level BLS required within 90 days of hire.
• Certification or Registration as a Medical Assistant preferred.

Nice-to-haves

• Experience in outpatient, ambulatory care, and/or community healthcare.
• Experience in care coordination and managing chronic conditions.
• Bilingual skills preferred.
