Responsibilities

  • Conduct holistic age-specific patient/family assessments upon visit according to defined standards.
  • Collect and document data related to patient's physical, mental, developmental, emotional and social condition.
  • Demonstrate age-specific competency in assessment of patients and interpret information based on their needs.
  • Assess family expectations for involvement in the assessment and continuous care of the patient.
  • Communicate the plan of care to the patient/family for their understanding.
  • Identify the need for MD involvement or potential changes in the plan of care.
  • Complete ongoing patient/family assessments according to appropriate clinical practice guidelines.
  • Facilitate timely communication between patient/family and other healthcare disciplines.
  • Conduct chart audits to meet JCAHO and organizational expectations.
  • Follow up on tests and lab results within established time frames.
  • Consider cultural, cognitive, emotional, and social influences on patient/family readiness to learn.
  • Gather patient information concisely and convey it to the MD.
  • Assist MD in clinic to maintain workflow and meet customer expectations.
  • Demonstrate effective delegation techniques and perform special tasks unique to Ambulatory Care clinics.
  • Coordinate patient care to ensure continuity of care.

Requirements

  • RN License (State Licensure and/or Compact State Licensure)
  • BLS Certification
  • PALS Certification
  • Bachelor's degree in Nursing or Associate's degree from an accredited school of nursing
  • Minimum 1 year of RN experience
  • Experience with pediatric patients (preferred)
  • Previous experience with EPIC (preferred)
