About the position

The Clinical Care Transition Educator is responsible for providing clinical education and training to ensure the successful transition of patients to Option Care Health's care delivery model. This role involves partnering with the sales team to increase patient transitions and conducting assessments to evaluate patient viability for the program.

Responsibilities

• Evaluate, educate, and train patients, caregivers, and facility staff about OCH services and products.
• Conduct patient assessments to determine viability for the OCH care delivery model.
• Partner with the sales team to understand educational needs and develop improvements to increase patient transition volume.
• Maintain confidentiality of patient and proprietary information.
• Document education using OCH applications and technology.

Requirements

• Active and unrestricted Registered Nurse (RN) license required.
• Minimum of 2 years of experience in the healthcare industry.
• Experience establishing and maintaining relationships with individuals at all levels of the organization.
• Knowledge of standard practices for services offered and relevant standards (i.e. ACHC, URAC).
• Experience providing customer service to internal and external customers.
• Basic PC skills including Microsoft Word, Excel, and PowerPoint.
• Experience in identifying operational issues and recommending strategies for improvement.
• Access to reliable transportation for local travel.

Nice-to-haves

• Hands-on home or alternate site infusion or discharge planning experience.
• Experience growing service provider partnerships.
