About the position

AMFM Healthcare is seeking a Clinical Case Manager to enhance the quality of clinical outcomes and client satisfaction at our adult residential mental health facilities. The role involves developing person-centered care plans, leading group therapy sessions, and managing client transfers and discharges. The ideal candidate will have a passion for improving lives through compassionate care and a commitment to evidence-based treatment protocols.

Responsibilities

• Develop person-centered care plans including prioritized problem lists and measurable objectives.
• Identify and prioritize individual needs and problems for clients.
• Be responsible for the client's aftercare plan and make decisions in their best interest.
• Lead and provide group therapy sessions utilizing Evidence Based Practices.
• Manage a caseload of up to 12 clients with weekly aftercare planning sessions.
• Coordinate and manage client transfers to different levels of care and discharges.
• Communicate with aftercare programs to facilitate care coordination.
• Initiate and develop comprehensive transition and discharge plans for clients.
• Prepare clinical documentation accurately and within agency time frames.
• Intervene during crisis situations as needed.
• Supervise and participate in the agency's Quality Management system and staff training.
• Provide accurate assessment and referral when appropriate.
• Adhere to ethical and legal standards of clinical practice.

Requirements

• Master's degree in a relevant field preferred; Bachelor's degree minimum.
• Active registration with BBS as an AMFT, ACSW, APCC, or fully licensed; or current CADC I or II certification.
• Previous case management or clinical experience preferred.
• Experience working with individuals with substance use disorders or mental health diagnoses.
• Valid driver's license.

Nice-to-haves

• Experience in addiction counseling.
