About the position

The Clinical Case Manager for Behavioral Health at CVS Health is responsible for utilizing advanced clinical judgment and critical thinking skills to facilitate member access to physical and behavioral healthcare. This role involves comprehensive assessments, care planning, and coordination of services to ensure optimal health outcomes for members. The position is fully remote and operates during standard business hours, Monday through Friday, with no weekend or holiday work required.

Responsibilities

  • Conduct comprehensive assessments of referred members' needs and eligibility.
  • Determine approaches to case resolution by evaluating members' benefit plans and available programs/services.
  • Incorporate strategies to reduce risk factors and address complex clinical indicators impacting care planning.
  • Perform crisis intervention for members experiencing behavioral health or medical crises and refer them to appropriate clinical providers.
  • Provide follow-up support to ensure members receive appropriate treatment and services.

Requirements

  • 3 years of direct clinical practice experience post master's degree in a hospital or alternative care setting.
  • Knowledge of mental health and substance abuse disorders.
  • Ability to communicate effectively while multitasking (talking on the phone and typing simultaneously).
  • Experience using Motivational Interviewing and good technological skills.
  • Valid unrestricted independent professional behavioral health clinical license in the state of residence.
