About the position

The Clinical Case Manager at Hope Valley Recovery is responsible for providing ongoing professional, technical, and clinical support through comprehensive assessment, planning, and treatment. The role focuses on coordinating care and addressing individual patient needs, ensuring effective communication with patients and their families throughout the treatment process.

Responsibilities

  • Meet with patients and families upon admission to explain the treatment facility's phase process and rules.
  • Serve as the primary point of contact for patients to coordinate their treatment, including internal and external appointments.
  • Communicate with patients' families weekly to update them on clinical care and discharge planning.
  • Facilitate groups and support the Clinical Treatment Team as needed.
  • Coordinate and implement discharge planning with input from the Clinical Team, patient, and family.
  • Review and finalize appointments for patients' discharge plans.
  • Provide referrals and assistance for disability and community support programs.
  • Meet face-to-face with each patient on the case load for at least 1 hour per week to review treatment and discharge plans.
  • Act as a clinical liaison for referral sources, providing updates on admissions and discharges.
  • Arrange for the transfer of care to other facilities when necessary during treatment.
  • Maintain regular charting and notes in the electronic medical records system.
  • Provide patients with schedules and information for upcoming appointments.
  • Attend weekly Clinical Staffing meetings and participate in case reviews.
  • Assist with crisis management and clinical intervention as needed.

Requirements

  • Minimum of a high school diploma or GED equivalent.
  • CDCA certification in the state of Ohio.
  • Minimum of 2 years' experience in the field of addiction.
  • Excellent oral and written communication skills.
  • Strong interpersonal competencies and professional behavior.
  • Very good organizational skills and strategic planning abilities.
  • Ability to prioritize and manage time efficiently.
  • Analytical thinking and attention to detail.
  • Patient-oriented with compassion and empathy.
  • Ability to work under pressure and perform well without close supervision.
  • Self-motivated and dynamic with a positive attitude.
