About the position

The Clinical Case Manager for Heart Failure Services at Penn State Health is responsible for coordinating interdisciplinary professional care for a select patient population. Responsibilities

• Coordinate interdisciplinary professional care for a select patient population.
• Develop individualized plans of care based on clinical pathways and managed care guidelines.
• Mobilize resources and manage systems to respond to data analysis and identify variances.
• Participate in developing programs to improve clinical quality and fiscal outcomes.
• Maintain knowledge of clinical systems and fiscal data related to specific patient populations.

Requirements

• Two years of nursing experience.
• Currently licensed to practice as a Registered Nurse in the state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
• AHA BLS certification prior to the end of the orientation period.

Nice-to-haves

• Experience as a case manager, in discharge planning, or with insurance issues.
• Appropriate certification for specialty area.
• Proficiency in Microsoft Office.
• Motivational interviewing skills.
