About the position

The position is responsible for coordinating interdisciplinary professional care for a select patient population, guided by individualized care plans, clinical pathways, and managed care guidelines. The role involves mobilizing resources, managing systems, and analyzing data to identify variances, while also participating in the development of programs aimed at improving clinical quality and fiscal outcomes.

Responsibilities

  • Coordinate interdisciplinary professional care for a select patient population.
  • Develop and manage individualized plans of care and clinical pathways.
  • Analyze concurrent data to identify variances and mobilize resources accordingly.
  • Participate in developing programs to improve clinical quality and fiscal outcomes.
  • Maintain knowledge of clinical systems and fiscal data related to specific patient populations.

Requirements

  • Two years of nursing experience.
  • Currently licensed to practice as a Registered Nurse in the state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
  • AHA BLS certification prior to the end of the orientation period.

Nice-to-haves

  • Experience as a case manager, in discharge planning, or with insurance issues.
  • Appropriate certification for specialty area.
  • Proficiency in Microsoft Office.
  • Motivational interviewing skills.
