About the position

The Clinical Case Manager position is a remote role focused on providing high-quality, child and family-centered services to foster-adopt families and children. The primary objective is to develop comprehensive child service plans that address the strengths and needs of the child, including aspects of physical, mental, emotional, and educational well-being. This role requires strong clinical assessment and organizational skills to ensure effective service delivery until permanency is achieved.

Responsibilities

• Develop the initial 30-day child plan of service and subsequent plans every 90 to 180 days until permanency is reached.
• Conduct clinical assessments to identify the needs of children and families.
• Write detailed treatment plans and service documentation.
• Communicate effectively with families and stakeholders to ensure service delivery.
• Manage time efficiently to meet deadlines for service plans.

Requirements

• Bachelor's degree in social services or a related human service field from an accredited college or university.
• 2-3 years of documented full-time work experience in therapeutic services for children and youth with special needs.
• Strong writing skills for treatment plan documentation.
• Excellent organizational and time management skills.
• Effective communication skills.
