About the position

The Clinical Case Manager I (CCMI) is responsible for providing clinical and case management services to residents living in permanent supportive housing for seniors and disabled individuals who have previously experienced homelessness. The role emphasizes the principles of Harm Reduction and Trauma-Informed care, engaging a caseload of approximately 15 residents, including adults, families, and children with histories of homelessness and mental health diagnoses. The CCMI will work to stabilize residents and assist them in transitioning to permanent housing placements while also providing training and support to agency staff.

Responsibilities

  • Identify and provide appropriate clinical/intensive case management services to residents with complex behavioral health issues using a Harm Reduction framework.
  • Support participants in stabilizing and ultimately transitioning to permanent housing placements.
  • Create and offer trainings to agency staff to improve clinical understanding and effectiveness of service provision.
  • Lead responder for crisis intervention, conflict resolution, and de-escalation services.
  • Identify issues that interfere with the delivery of services and develop techniques to resolve them.
  • Identify, develop linkages with, and coordinate access to community-based services for residents; accompany residents to community-based appointments as appropriate.
  • Make clinical determinations and facilitate 5150s as needed.
  • Conduct discharge planning for residents from inpatient and residential care environments and coordinate with on-site resident services staff.
  • Complete documentation per agency policy and actively support contract reporting, data collection, record keeping, and chart reviews.
  • Contribute to case conferences and other meetings through presentation of cases, discussion of clinical issues, and exchange of information and ideas.

Requirements

  • ASW/AMFT/APCC certification.
  • Demonstrated ability in providing effective clinical services within a context of harm-reduction practices and voluntary services.
  • In-depth knowledge of harm reduction practices.
  • Strong clinical observational and assessment skills.
  • Experience in providing services to individuals struggling with substance use and mental health issues or who have experienced homelessness.
  • Experience in engaging residents, crisis intervention, conflict resolution, and meeting facilitation.
  • Experience in creating and conducting trainings.
  • Knowledge of supportive housing, housing first, trauma-informed care, motivational interviewing, and harm reduction principles.
  • Proven ability to navigate San Francisco's behavioral health care system and experience in advocating for access to services, including collaboration with other agencies.
  • Excellent computer skills (Outlook, MS Word, Excel, Salesforce or other cloud-based database).
