About the position

The Clinical Case Manager plays a crucial role in assisting clients with complex needs by coordinating access to medical, social, educational, and other essential services. The primary objective is to enhance the functioning of individuals through effective case management, ensuring they achieve self-sufficiency and satisfaction in their chosen living, learning, work, and social environments.

Requirements

• Bachelor's degree in psychology or related field preferred
• High School Diploma required
• Minimum three (3) years experience working with adults with mental illness
• Minimum two (2) years experience in case management or mental health discipline
• Successful completion of Level II background screening
• Demonstrated knowledge, abilities, and skills essential to this position
• Excellent organizational and time management skills
• Good judgment and common sense
• Ability to speak and comprehend functional English
• Good reading and writing skills
• Great verbal communication skills
• Good analytical skills
• Good computer skills
• Excellent stress management skills
