About the position

The Clinical Case Manager - LPN is responsible for providing remote clinical care to patients with multiple chronic health conditions. This role involves creating patient-centered care plans, reviewing clinician documentation from telehealth visits, and engaging with patients and healthcare providers to improve health outcomes and reduce hospitalizations.

Responsibilities

• Create a care plan for new patients and identify barriers related to care plan goals.
• Update care plan goals annually.
• Establish interventions for case managers to address patient care gaps.
• Determine appropriate remote patient monitoring devices and interventions.
• Educate patients, family members, and caregivers about care plans.
• Engage with healthcare providers to reduce emergency room visits and hospitalizations.
• Address escalated patient vitals and refer to clinical staff as necessary.
• Submit organized and timely progress notes and documentation.
• Provide outreach and ensure clinical follow-up services are delivered.
• Provide crisis intervention when necessary.
• Assist with education and training of case management staff.
• Provide clinical training for case management staff.
• Perform other related duties as assigned.

Requirements

• Active LPN state license in good standing in New York.
• Multiple state licenses are preferred.
• Bilingual in Creole and English.
• Experience in virtual care/telemedicine.
• Experience working with the geriatric population and homecare agencies.
• Experience providing clinical services to patients with chronic health conditions or disabilities.
• Effective interpersonal, communication, and teamwork skills.
• Experience with harm reduction and motivational interviewing techniques.

Nice-to-haves

• Fluency in Spanish.
