About the position

The Clinical Case Manager is responsible for planning, organizing, and coordinating swing bed admissions, as well as reviewing emergency department admissions and discharges. This role involves providing administrative and clerical services, contributing to process improvement, and managing medical records in collaboration with referral specialists. The position operates under the general supervision of the Director of Nursing and requires availability on weekends as needed.

Responsibilities

• Plan, organize, and coordinate swing bed admissions.
• Review emergency department admissions and discharges.
• Provide administrative and clerical services.
• Input into process improvement initiatives.
• Coordinate with referral specialists.
• Manage, obtain, and share medical records.
• Perform financial assessments for in-patient stays.
• Respond to queries from doctors' offices, families, and outside facilities.
• Provide case management, social services, utilization review, and discharge planning.
• Identify and coordinate swing admissions and verify admission requirements.
• Review ED admissions/discharges and coordinate follow-up visits with PCPs.
• Develop and maintain relationships with specialty offices.
• Participate in interdisciplinary team meetings and UR meetings.
• Review patient charts for counseling requests and assess needs for intervention.

Requirements

• Three to five years of experience in clinical nursing.
• One to two years' experience in case management or related field preferred.
• Current Licensure to practice in the state of California.
• Bachelor's degree in nursing highly preferred.
• BLS certification required, ACLS preferred.

Nice-to-haves

• Bilingual in Spanish preferred.
• Proficiency in using computers and various software systems including Microsoft Office, Athena, EMR, and Point Click Care.
• Ability to multitask and work effectively in a fast-paced environment.
• Knowledge of community resources and support services.
