About the position

The Clinical Coordinator II - RN at Columbia University is responsible for managing patient care within the Division of IBD Surgery. This role involves initiating patient assessments, developing educational programs for pre- and post-office visits, and ensuring quality patient-centered care. The coordinator will oversee case management, coordinate diagnostic testing, and facilitate home care services, all while supporting ongoing quality improvement initiatives within the division.

Responsibilities

  • Assess health status by completing a nursing assessment.
  • Perform telephone triage for two physicians.
  • Notify physician/LIP of abnormal findings.
  • Establish a care plan in collaboration with the patient, family, and health care team.
  • Collaborate with other care coordinators to ensure smooth and effective transitions of care.
  • Assist with specialized diagnostic and therapeutic procedures.
  • Assist with specialized patient care equipment as required by the clinical department and medical specialty assigned.
  • Administer medications and treatments (including infusions) as ordered by physician/LIP and monitor patients for response.
  • Initiate CPR and other emergency measures.
  • Cover other nursing roles within the department.
  • Provide health education to patients and families, developing, evaluating, adapting, and documenting education to learning needs.
  • Assist in preparation of department-specific education for patients and colleagues.
  • Facilitate smooth operation of the ambulatory areas and discharge of patients from the Outpatient Endoscopy area as needed.
  • Coordinate care for IBD and colorectal patients, including those from out of state, to streamline office visits, testing, imaging, endoscopy, and surgery.
  • Participate in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience, and patient safety.
  • Maintain professional growth and development in the nursing field and identify annual core competencies.
  • Other duties as assigned.

Requirements

  • Bachelor's degree or equivalent combination of education and 4 years work experience required.
  • New York State RN license required.
  • BCLS certification required.
  • Intermediate MS Office skills.
