About the position

The Clinical Coordinator at DaVita is responsible for delivering high-quality care to patients in an outpatient dialysis center. This role involves coordinating patient care, ensuring safety and comfort, and building meaningful relationships with patients and their families. The Clinical Coordinator will work closely with a team of clinicians to manage patient observations and treatment plans, particularly for those with complex health needs.

Responsibilities

• Coordinate ESRD patient care and services, including tracking lab values and vital signs.
• Ensure the clinical team delivers the highest level of care to patients.
• Build meaningful relationships with patients and their families.
• Oversee a group of patients with the support of Patient Care Technicians (PCTs).
• Conduct patient observations and measure vital statistics.
• Manage machine setup for dialysis treatments.

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Current CPR certification required.
• At least 18 months of registered nursing experience.
• Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
• Associates Degree in Nursing (A.D.N) from an accredited school of nursing required; B.S.N preferred.
• Basic computer skills and proficiency in MS Word and Outlook required.

Nice-to-haves

• Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
• Supervisory experience preferred.
• Experience in ICU, CCU, ER, or Med Surg preferred.
